To make sure you are able to receive information and magazines (if you subscribed) that will come to you as part of your USUA membership, USUA offers an easy way to make address changes or modifications online.

To change your address please complete all fields then click the "Send Form" button at the bottom of this form. Those fields in bold are required fields.

Allow 7 to 10 business days for changes to be implemented. Please allow 6 to 8 weeks for magazine subscription address changes to take effect.
